Develop a Client Care Profile
When you call Traditions Cares, we start by gathering information on how we can best help you. We'll ask some questions to get a better understanding of your wishes. We then arrange an in-home visit to perform a basic needs and environment assessment to determine the type of care you need. Whether you need companion, personal, nursing or specialty care, we are there to answer all of your questions and provide you with the services you need.
Create a Personal Care Plan
Our next step is to create at Traditions Cares plan that is centered to your needs. This incorporates you and your loved ones needs, routines, and preferences to create a supportive plan that provides you with the most benefits.
We then review the Traditions Cares plan with you in detail and modify it as you see fit.
Identify the Best Caregiver for You
Allowing someone in your home is a big step. We are honored and grateful that you put your trust in Traditions Cares, so it is important that we make the perfect caregiver or skilled nurse match. We consider numerous factors such as interest, personalities, and then home medical support services needed. All caregivers and healthcare professionals are screened including background and reference checks. They are also bonded/insured. Of course your comfort matters the most so if you ever have any concerns, we will work to find you a new caregiver.

Traditions Cares caregivers keep detailed notes in exactly what happened each time they provide service. Supervisory staff also make periodic visits to your loved ones homes to ensure things are going well. Because your care team is trying to notice and report changes in health or condition, we can quickly adjust care if needed.

At TRADITIONS CARES, patients are cared for by an expert team consisting of a nurse, doctor, assistant, social worker, and chaplain. Other therapeutic specialists, such as a music therapist or a trained volunteer, may assist the team in meeting the emotional and spiritual needs of the patient and his or her family. Our specialists create individualized care plans to promote comfort, dignity, and life quality.
Most hospice care is provided in the patient's home, as this is where the majority of terminally ill patients prefer to be: in familiar settings with familiar routines and familiar faces.
